Browse Forms is for VIEWING the application questions. Sub-sections of the application can be expanded/viewed by selecting Yes / No answer to questions within the application. You must create a user account and login to your user home page to complete an application form.
  • If space is insufficient for any answer, type your answer in a separate word processing document, print, and attach that sheet to this application
  • Err on the side of over-disclosure
  • Answer every question that is applicable to you. If a question is not applicable to you, answer the question “Not Applicable”
  • Include original of the signed Declaration, Release and Authorization Form
  • Provide the full number, street name, city, state and zip code for each address.
  • If an answer to any question changes before your admission, you are obligated to update your application.
  • Complete all the forms required
  • Sign all the forms requiring your signature
  • No exceptions to the filing deadlines will be granted.
  • Applications will not be considered complete or properly submitted unless the applicant has first or simultaneously paid the required examination application fee. A link to the online payment portal for this fee can be found here: https://ebiz.osbar.org/ebusiness/ProductCatalog/Product.aspx?ID=1. Member applicants do not need to pay a late fee, even though they will be submitting their application after the timely filing deadline.
  • If you withdraw in accordance with Rule 4.10(1)(g), you will receive a partial refund.
  • If the Member Seating Capacity is reached before the filing deadline, we will announce the member application closing on the OSB Admission Homepage.
